Our 2012 donation stationery is now available. There are a number of ways you can get an application form:
Collect a form
Trust Cottage, next to Trust House, 2 London St, Hamilton.
Post a form
Phone us on 0800 436 628 or
Download a writeable form
To download an application form to your computer, click on writeable PDF version of the form. Save a copy of the form on your computer. You can then begin working in it. Once completed you will need to print it off, sign it and send it to us at PO Box 391, Hamilton 3240 or deliver it to Trust Cottage, next to Trust House, 2 London Street, Hamilton.
These are NOT online application forms and we cannot accept emailed or faxed applications.
Print a web form
Open the PDF printable version of our application form. Print it off and hand write your answers on to the form. Sign it and send it to us at PO Box 391, Hamilton 3240 or deliver it to Trust Cottage, next to Trust House, 2 London Street, Hamilton.
Please ensure you attach the correct postage to adequately cover the size and weight of your mailed application. If you are unsure, contact New Zealand Post on 0800 501 501, or visit your local PostShop.
Please note: advertised closing dates will be strictly adhered to and late applications will not be considered.
