To download the 2010 forms - including the new multi year funding application - click on the application form in word or the pdf versions in the orange box on this page.

Please note: these are NOT online application forms and we cannot accept emailed or faxed applications.    

 

 We have developed two forms for you:


Version 1: Type on Form

  • Save the Word format file from the orange box to your computer
  • Type your answers directly on to the form
  • Print it off
  • Sign it, add attachments and send it to us at P.O. Box 391, Hamilton 3240 or deliver it to Trust Cottage, 2 London Street, Hamilton 


Version 2: Handwrite on Form

  • Open the pdf file listed in the orange box 
  • Print it off
  • Hand write your answers on to the form
  • Sign it, add attachments and send it to us at P.O.Box 391, Hamilton 3240 or deliver it to Trust Cottage, 2 London Street, Hamilton 

 Note that the advertised closing dates will be strictly adhered to and late applications will not be considered.

 

Please ensure you attach the correct postage to adequately cover the size and weight of your mailed application.  If you are unsure, contact New Zealand Post on 0800 501 501, or visit your local PostShop.
